This report is a clinical review of. 103 patients of gastroduodenal perforation which were experienced in the department of general surgery at Inchon Christian Hospital from January, 1972 to December, 1976.
1. Among the 103 cases of the series of disease were gastric ulcer 75 cases (72.8%) duodenal ulcer 22 cases (21.3%) stomach cancer 2 cases (1.9%), others (drug, stress, trauma) 4 cases (3.8%).
2. The sex incidence was male 92 cases to female 11 cases, sex ratio:M:F=8.4 : 1
3. The age incidence occur in any age but the peak incidence was third decade. The older age: 69 year, the younger age 6 day, average age: between 38 and 39 year.
4. Monthly the highest distribution of gastroduodenal perforation 33 cases (32%) at autumn but showed no differences.
5. Among these group, 25 cases (24.3%) of patients had a history of ulcer symptoms over 1 to 2 years and 21 cases (20.4%) had below 1 year.
6. Analysis of subjective symptoms revealed in order of indigestion 87 cases (84.5%), epigastric pain 79.6 percents.
7. Positive findings of physical examination included as follows epigastric tenderness were found in 93 cases (90.3%), rebound tenderness and rigidity were 97%, 94.2%
8. Demonstration of pneumoperitoneum by plain X-ray was revealed in 87, cases (84.5%).
9. Distribution of blood type was 0 type. (30.9%), A type (26.2%).
10. Among the 81 cases of gastric perforation, 44 cases (42.1%) was pylorus, antrum. 20 cases (19.4%), body 7 cases. Twenty-two cases (27.4%) of duodenal perforation was at 1st portion of anterior surface of duodenum.
11. The operative procedures in 103 cases as follows; 49 cases (47.5%) was simple closure, 35r cases (34%) were immediate gastrectomy, 10 cases (9.7%) were vagotomy and antrectomy, or
vagotomy.and hemigastrectomy, 6 cases (8.7%) were vagotomy and pyloroplasty.
12. All mortality rate was 4.8%.
